IT infrastructure and exchange capabilities within communities, positioning each community to pursue a new level of sustainable health care quality and efficiency over the coming years; 2. Translating investments in health IT in the short run to measureable
improvements in cost, quality and population health; and 3. Developing innovative approaches to performance measurement, technology and care delivery to accelerate evidence generation for new approaches. The proposed panel will be a moderated discussion hosted
by a leader of the ONC Beacon Community Program, and 3-4 Beacon Community leaders. The diversity of populations, clinical priorities and partnerships represented in the program will be reflected by the panelists (i.e., rural vs safety net, chronic disease
